Care notes that see what the sensors saw.

What a care note contains

Every care note in Halo has:

• Structured observation — free-text plus tagged categories (nutrition, hydration, sleep, mood, mobility, medication, incident)
• Sensor context — the sensor stream at the time of the note is linked automatically (temperature, activity, sleep, alerts fired that shift)
• Care-plan reference — the current care plan and any goals or interventions active at the time
• Attribution — who wrote it, when, from which device
• Reactions and follow-up — read receipts from other staff on the shift, tagged actions with due dates
• Consent-aware share flags — whether the note is visible to family via the family portal, based on the resident’s consent

Free-text notes remain free-text. The structure is there to help retrieval and reporting, not to force staff into rigid forms.

Immutability and correction

Care notes are immutable once submitted. This is by design:

• If a note is wrong, staff append a corrective note that references the original
• The original note remains visible with a “corrected — see note X” flag
• Both notes are attributed and timestamped
• The audit trail records that a correction was made and by whom

This matches CQC and clinical record-keeping expectations: nothing silently rewrites history. Corrections happen in the light.

Consent-aware family access

Family members can be invited to see care notes tagged as shareable. This is entirely opt-in:

• The resident decides what’s shareable, on installation and any time after
• Categories can be scoped — some residents share “activity and appetite” but not “mood” or “incident”
• Family members are named — not a generic family login; each invited person is auditable
• Access can be revoked any time — with or without warning
• End-of-life scenarios — configurable in advance so consent state at the moment of a resident’s decline is documented

Care-plan integration

Care notes reference the current care plan. When care plans update:

• Existing notes retain their historical care-plan reference (nothing rewritten)
• New notes reference the new plan
• Plan changes themselves are auditable — who changed what, when, and why

Care plan review meetings can pull the last 30 or 90 days of notes filtered by category or intervention, with the sensor stream and alert history alongside. Reviews stop being about “what the staff remember” and start being about what actually happened.

Search and reporting

Care notes are searchable across:

• Date range, resident, staff member, category
• Free-text keyword search
• Alert-linked notes (all notes that reference a specific alert)
• Care-plan intervention (all notes referencing a specific plan goal)

Standard reports:

• Daily shift handover report (grouped by resident)
• Weekly care review pack (for a specific resident)
• Monthly compliance report (audit trail summary)

All exportable as PDF or CSV. Enterprise tier includes report scheduling to a nominated distribution list.
